of North America. If you are interested, please subscribe by
filling out the form below or by printing out the PDF file and
mailing it to us.

If you are not a sponsor, please send us $25 for the cost of
shipping and handling of the CD/DVD (you only pay for shipping
and handling and not the actual cost). If you have MQI-NA sponsorship, you are not required to do this because the sponsorship fee includes the
CD-Subscription.
